There's no doubt that the idea of a mouse oriented in this is a good idea, and helped with a lot of shoulder and arm pain I was having.
But I've had nothing but problems with this thing. After about a month of use, it started to behave erratically. The page scrolls up when I'm scrolling the wheel down (but only sometimes), or I roll just one little tick on the wheel and the page jumps to the bottom. I thought the wheel perhaps needed cleaning, but there is no way to open the mouse, no screws, no obvious place where it could be pried open without breaking it.
The most recent problem is that the mouse turns itself off after about 30 seconds (but only sometimes) -- and I mean completely off. The only way to get it to work is to unplug it from the USB port and plug it back in, which I am now doing at least a dozen times a day.
All of this would be fine if the company offered to replace it, or at least offered some decent technical support. But they are really missing in action here. E-mails go unanswered. A phone call put me into voice mail which was never returned. Obviously the other reviewers here really like this mouse, and I would too, if mine worked.
Buy it if you want, and hope you get one of the good ones.

I am a bit of a mouse junkie, I have purchased about 10 mice/trackball in the last 2 years. I am in search of the best and most usable mouse for work. I am on the computer 12+ hours a day most of the week.
I will rank this mouse a close #1 for these:
1. Very lite, easy to move.
2. Excellent angle, not as upright as the "Evoluent VM3R2-RSB Vertical Mouse 3" which I also have in my arsenal of mouse.
3. The back and forward buttons are easily reachable with the thumb.
4. Easy to click.
5. Thumb area has a hook area where your thumb can sit which the Evoluent doesn't have is a great plus+
6. The mouse look nice, perfect blend of shiny and dull rubbery areas for better control.
7. Overall feeling of my hand and arm don't feel tired after a full days use, that is very important to me.
The mouse feels like it should be held with a full palm, but my hand is too big and misses the buttons, therefore this is the only downside, I have found a position that is comfortable and doesn't set the side of my hand scraping the desk (like the Evoluent did).
One note: This is my true #1 input device: Kensington Turboring which has been discontinued & mine broke is the best trackball I have used. I wish Kensington made it again. My thumb was rarely used which I believe can cause most of the strain on the hand and arm.
UPDATE: After 1 year of use, the mouse is still my daily driver. I tried a different mouse (3M Ergo which was a fail). The mouse still works great, the only downside after 1 year of use is the scroll wheel now makes a squeaking sound when used, not unbearable but can get annoying if the room is quiet.

I felt the difference in my right wrist instantly. According to websites, "Ergonomic design keeps the arm positioned for natural, comfortable movement. Ever notice that Gamers don't have carpel tunnel syndrome like most typists even though they likely spend more time at the computer? Well, 3M noticed! Your wrist sits on this ergonomic mouse like it does on a video game joystick. The weight of your hand is no longer carried by your wrist. Rather, your hand is relaxed on the mouse.
The relief in pressure on your wrist is instant. The cost may seem high, but it's less than a chiropractic appointment. Comes in 2 different sizes (small/medium and medium/large). This is a RIGHT handed mouse. You can program the left/right clicks if you are left handed, but the "joystick" is angled, so it's not as wonderful if you are a lefty.
Added note: The above review was written 2 years ago. It has been 3 years that I have been using this mouse. My wrist pain is completely gone. This mouse is so worth it. That being said, it is a P.O.S. as others have mentioned: the mouse does not travel well (it breaks if not protected in your computer bag) and breaks when dropped on hard surfaces -- I put a carpet under my desk and bought a glue gun to cut down on mouse costs. After gluing them back together, 3 have acquired the "double click" disease. Even with these drawback, I still feel that this mouse is worth it. I have purchased 8 over the past 3 years - I now keep one at work, one on my desk at home, and a "floater" for my home laptop that moves from room to room but does not leave the house unless carefully wrapped in it's original box. For "daily travel", because the 3M mouse has helped my wrist heal, I am now able to use a regular mouse, so I now keep a regular mouse in my backpack.
